Professional Educational Services Group, LLC requires the premium payment of Dental, Vision, Life and Disability on an electronic payment basis. To participate in these benefits simply fill out and submit an Automatic Payment Application along with a blank, voided check or deposit slip from your designated account for verification. Please allow up to 3-4 weeks for processing.
